Your Local Community Pharmacy Rewards LoyaltyOne Loyalty Program Terms and Conditions
By applying for and/or using your Your Local Community Pharmacy Rewards LoyaltyOne card (Card) or claiming any rewards points or participating in the Program, you agree to be bound by these Terms and Conditions, including any variation to them from time to time.
Reference to “we”, “us” or “our” is a reference to the participating Your Local Community Pharmacy where you apply for membership (Your Local Community Pharmacy).
1. Becoming a Member
1.1. To become a member (Member) of the Your Local Community Pharmacy Rewards LoyaltyOne Program (Program), you must be an individual, over 18 years of age, have a current Australian residential address and complete and return this application form to Your Local Community Pharmacy. Once your application is accepted, you will be issued with a membership card (Card) from this Your Local Community Pharmacy.
1.2. Membership is not transferable across Your Local Community Pharmacy stores. Points can only be earned and redeemed at the individual Your Local Community Pharmacy where you apply for membership (Your Local Community Pharmacy) unless otherwise stated on your Card.
1.3. We may refuse your application for Membership for any reason.
1.3. Membership is free (there is no joining fee) however, we reserve the right to charge for any replacement or additional Cards.
1.4. You may terminate your Membership of the Program at any time by contacting the Your Local Community Pharmacy at which you signed up, or by emailing [email protected].
1.5. It is your responsibility to ensure that your personal details are kept up to date and to advise Your Local Community Pharmacy if your Card is lost or stolen.
2. Earning Points
2.1. Points can be earned by presenting your Card when you make an Eligible Purchase (non- prescription items and non-Exempt Items) at Your Local Community Pharmacy.
2.2. Points cannot be earned on “Pharmacy Only”, “Pharmacist Only”, or prescription medicines, or as otherwise advised by Your Local Community Pharmacy (Exempt Items).
2.3. You will earn 1 point for every $1.00 spent on Eligible Purchases unless otherwise stated by Your Local Community Pharmacy.
2.4. From time to time, you may be able to earn additional points (Bonus Points) in relation to special promotions as advertised from time to time by Your Local Community Pharmacy
2.5. Points are not transferrable and not redeemable for cash and may expire as determined by Your Local Community Pharmacy. Points can only be redeemed at Your Local Community Pharmacy.
2.6. Any points issued as a result of fraud, dishonesty or error, or that relate to a purchase that is subsequently cancelled or refunded will be deducted from your Points balance.
3. Using Points
3.1. Points can be redeemed in part or full payment for purchases up to the amount of the Points balance available on your Card.
3.2. No cash refund will be made where the Points balance on the Card exceeds the price of the items purchased.
3.3. Points can only be redeemed by the person whose name the membership is in.
3.4. Points cannot be redeemed on Exempt Items (ie “Pharmacy Only”, “Pharmacist Only” or prescription medicines (or as otherwise advised by Your Local Community Pharmacy).
4. Privacy
4.1. We collect, use and disclose your personal information to enable you to be able to participate in the Program and to provide you with information and offers through the Your Local Community Pharmacy Rewards Program. Any personal information collected by Your Local Community Pharmacy will be handled in accordance with its Privacy Policy that is available from Your Local Community Pharmacy .
4.2. By registering as a Member and participating in the Program, you acknowledge that you have read and understood our Privacy Policy and consent to your personal information, including health information being disclosed to Symbion Pty Ltd (ABN 25 000 875 034) and Intellipharm Pty Ltd (ABN 92 001 235 374). Symbion will handle any information disclosed to it in accordance with its privacy policy which can be found at https://www.symbion.com.au/privacy-policy/. Intellipharm will handle any information disclosed to it in accordance with its privacy policy which can be found at https://intellipharm.com.au/privacy-policy/. You agree and consent to information being disclosed to Symbion and/or Intellipharm for the operation of the Program and for marketing, planning, product development, research and other purposes relating to the Program.
4.3. You agree that by registering and participating in the Program, we (including Symbion) may contact you from time to time by mail, phone, email, or SMS with marketing material (including in relation to products and services) about the Your Local Community Pharmacy Rewards Program, Your Local Community Pharmacy and its related businesses that may offer products and services.
4.4. To opt out of receiving marketing material, you can unsubscribe by updating your membership profile at www.loyaltyone.com.au or instore at Your Local Community Pharmacy.
5. Changes to the Program
5.1. We, (including Symbion and Intellipharm) reserve the right to change, suspend or terminate these Terms and Conditions or the Your Local Community Pharmacy Rewards Program at any time, without notice, even if this would reduce or eliminate the number of Points already issued or result in cancellation of Points already issued.
5.2. We will use our reasonable endeavours to notify you by posting a notice in Your Local Community Pharmacy.
5.3. Neither we (or Symbion and/or Intellipharm) will be liable to you for the termination of the Your Local Community Pharmacy Rewards LoyaltyOne Program for any reason, whatsoever, including for any Points on your Membership at the time of termination.
6. General
6.1. Any tax, liability or duty incurred by you arising from your participation in the Your Local Community Pharmacy Rewards Program is your responsibility.
6.2. Nothing in these Terms and Conditions limits your rights under the Australian Consumer Law.
